How to Apply for a Medical Marijuana ID Card in the State of Oregon.
Step 1:
Obtain a Medical Marijuana Certification from one of our Doctors
- * Patients’ Medical history is reviewed prior to appointment. If you do not have your documentation from your attending physician, we can help, simply complete our Medical Record Release Form below.
- * Attending Physician’s office has up to 30 days to respond once a request for patients’ records has been submitted.
Step 2:
Fill out the State of Oregon Medical Marijuana Program Application form.
Step 3:
Completed signed & dated application form
Step 4:
Submit documentation from attending physician that diagnoses qualifying debilitating medical condition
Step 5:
Pay all appropriate application dues & fees.
Step 6:
Make a copy of your Oregon Driver's License or ID Card as proof of residency.
Step 8
Obtain the required fees and attach to the application.
Step 9:
Submit all of the above to the State of Oregon Public Health Department.
